104: Pre and Post Op Care of the Orthopedic Patient

This 3-part series focuses on the two most common orthopedic surgical procedures, hip and knee replacement.  Program One focuses on the demographics and common causes related to hip fractures, post-injury assessment and management, the types of fractures and the surgical approaches used for each, and gives an overview of patient management from arrival at the emergency facility through discharge and home care follow-up.  Program Two provides detailed information on the surgical process and nursing care, including wound management, ongoing assessment based on a head-to-toe model, and use of the Five Ws when determining the cause of postoperative fever.  Discharge planning, patient and family teaching, and home care, including home safety modifications are also discussed.  Program Three focuses on knee replacement.  Nursing care and assessment is described, this time using a critical pathway or “timeline” model.  Assessment for common complications such as DVT and atelectasis are covered in detail, as is pain management.  Discharge planning includes discussion of the decision to send a patient directly home with home health follow-up, to a TCF, or to a long-term care facility.

4. Describe a systems approach to postoperative assessment

6. List the standard discharge goals.
7. Discuss the role of home health care in postsurgical management and appropriate home modification actions.
